Services

CWW_PCN_Logo_removed background.pngHealth & lifestyle coaching is a non-clinical, personalised care role. We help to support you to increase your ability to self-manage and motivate you to commit to changing your lifestyle and improve your health.

We work on behaviour change and focus on health-related outcomes, such as weight management, lowering blood pressure and cholesterol and improving mental health through making better choices (e.g., diet, exercise).

We will work with you to set personalised goals, change your behaviours and educate you over an initial 12-week period, leaving you with the ability to lead a much healthier lifestyle after finishing the plan. We focus on making small tweaks and achieving small goals every week, rather than making any drastic changes.

Every Body Counts

A free programme for children and young people aged 5-16 years* (up to 25 years with special educational needs) and their families. The Every Body Counts programme aims to empower young people with the knowledge, tools, and confidence they need to lead healthier lives now and in the future. Every Body Counts will offer a range of engaging activities and resources designed to support young people’s wellbeing through movement, nutrition, emotional wellbeing support, and family-focused activities.

*When child’s BMI measures over the 91st centile.

The Lifestyles Team are on hand to offer a range of weight-management support – including drop-in clinics, group and one-on-one sessions – whichever suits you best.

Lifestyles advisors will talk you through the programme and discuss your current eating pattern and what positive changes you can make to support you in making sustainable diet and lifestyle changes to improve your health.
